A well-designed and properly functioning faculty evaluation process provides a major communication link between the school system and faculty members. Thus, this study aims to develop a rating scanner app for Faculty Evaluation of DMMMSU using android mobile phone. Specifically, the study aims to determine the level of seriousness of the problems encountered in the existing procedures of faculty evaluation, determine the functional and non-functional requirements, design and build the FER Scanner application and determine its level of usability. The researchers used the descriptive and developmental research designs in accomplishing the study. The researchers conducted an interview and administered a survey questionnaires to the chairpersons and the deans of the DMMMSU-SLUC, whereas, a Mobile Application Development Life Cycle (MADLC) Model was utilized as a guide in developing the mobile application. Findings of the study are as follows; the respondents regarded that the level of seriousness of the problems encountered in the faculty evaluation of DMMMSU as Very Serious Problem; the Functional Requirements of the mobile application is to compute raw data gathered from the answered sheets and show the average scores per evaluator, and the evaluation process should be efficient and faster; and for the Non-Functional Requirements, it should generate and manage results from the scanned answered sheets, download the average scores from the evaluators, and print the results from the scanned answered sheets; the Mobile Faculty Evaluation Rating Scanner was developed using the Mobile Application Development Life Cycle Model to improve the university's performance in handling faculty's evaluation were it was regarded as Very Good in terms of its validity; the level of usability of the developed Mobile Faculty Evaluation Rating Scanner was evaluated Usable.
